Description
The Initial Health Consultation is for clients who have completed the complimentary strategy session, done some research, and are ready to begin the healing process with Live Well. This 90‑minute consultation with a functional medicine professional reviews your Comprehensive Health History, Nutritional Assessment Questionnaire (NAQ), and your Blood Chemistry Panel from a functional health prospective. You will also receive your first customized program with a review consult to begin your healing journey.
We use a functional medicine approach that looks at all factors influencing health—including the physical, biochemical, and emotional body. Together, we’ll explore your unique history, diet, and lifestyle to uncover clues behind your symptoms and co‑create a practical plan forward.

- Review & discussion of your Comprehensive Health History and Nutritional Assessment Questionnaire
- Insight to identify root causes behind your symptoms
- Assessment recommendations (any additional labs/info needed)
- Program recommendations (appropriate programs or classes to begin)
- Kinesiology exam either in person or via photo testing
- Custom wholesale lab panel or your lab work (within last 6 months) analyzed in a Functional Health Report

- Session delivered in person or via secure Zoom.
- You’ll receive a Functional Health Report and a customized program after the consult.
- Follow‑up program/class options will be discussed as needed.
Important Note Prior to Purchase
This program is non-refundable, non-transferable and must be completed within 60 days of purchasing.
Restrictions: (1) Patients must be 6 years old or older for test(s) that require any blood draws. Patients under the age of 18 require the parental/guardian consent form to be completed electronically. (2) We do not support lab ordering for patients in NY, NJ, and RI.
