Test Preparation and Reminders
Please make sure ALL your information is correct.
- Are you submitting a current PDF lab panel or will you have a blood draw from Lab Corp.
- If you choose a blood draw, you will receive a requisition form to have your blood drawn at
Lab Corp. - Please book your session review a minimum of 7 days out to allow time for the lab to process your specimen.
- Be ready to have your blood drawn as soon as possible.
Important Note Prior to Purchase
This program is non-refundable, non-transferable and must be completed within 60 days of purchasing.
Restrictions: Patients must be 6 years or older for a blood draw. Patients under the age of 18 require the parental/guardian consent form to be completed electronically. We do not support lab order for patients in NY, NJ or RI.
